--- RPG ---

Revised Core Rulebook (D20)
A bit imprecise...

Level shots
The Clone Wars
Mission selection screen
Battlefront 2
(The second image is not from the game.)

The Galaxy viewed from an unusual angle. Except from Hoth, none of the planets have been placed on other galaxy maps.
Rebellion
20 sectors, 200 systems, almost everything is incorrect on the map. Details here.
Empire At War
Why? Is it so hard to create an accurate galaxy map in 2006?
And Forces of Corruption does it again...
Galaxies
...
Knights of the Old Republic
Misplaced everything. The second image shows planets that were left out from the game.
Misplaced Telos. (Sith Lords) The third image shows planets that were left out from the game.

Top-down view galaxy map, shows the location of Tatooine/Geonosis. The Galaxy has only 2 spiral arms here...
Geonosis and Tatooine.
The Empire Strikes Back

"Even the placement of the luminous Rishi Maze on the Jedi Archive map has provided some conversation fodder for fans, perhaps shedding some light on just what astronomical phenomena Luke and Leia were looking at at the end of The Empire Strikes Back." - starwars.com

"Since that image (or one similiar from AOTC) has been used as the backdrop for published galaxy maps, I think it is pretty clear that it is the galaxy that you are seeing." - Leland Chee

This galaxy is rotating clockwise, so if it is the GFFA then the rebel fleet is somewhere below the ecliptic.
